Care Manager (CNA/CHHA) Full Time 3p-11p

The Caregiver will report to Health & Wellness Director. The Caregiver is a Full-Time and/or Part-Time, non-exempt, hourly position. Schedule: Full-Time 3-11pm Salary Range: $21/hr -$22/hr Reports To The Care Manager will report to the Health and Wellness Director and/or Designee. FLSA Classification: This position is classified as non-exempt and is an hourly position. Job Overview Assists residents in all aspects of their daily life as indicated in the resident’s care plan, including personal care, food service, housekeeping, laundry, behavior management, socialization, and activities. Responsibilities and Duties • Provide resident care to residents that include bathing, dressing, toileting, and feeding, as outlined in their care plan. • Respects the needs of team members and residents by adhering to schedule and documenting attendance properly. • Maintain the safety, comfort, dignity, and confidentiality of all residents. • Encourages and escorts (when needed) residents to meals and activities. • Ensures all assigned residents are clean, neatly dressed, groomed, safe, and fed during assigned shifts. • Document all services provided in EHR for your assigned residents during your shift. • Monitors and reports any change of condition in a resident immediately to your supervisor. • Appropriately utilizes and keeps electronic documentation “devices” safe and secure within the community; efficiently documents care/services provided during the shift. • Participates in and attends all in-service trainings as scheduled. • Ensures compliance with all Federal, State, and local regulations, as well as community policies and procedures. • Provides basic housekeeping and/or laundry services when needed. • May be asked to obtain weights and vitals of residents and document per RN or designee request (LPN in MA &PA). • Assist with self-administration of prescribed medication to residents and maintain related medical records, by way of queuing, prompting, and/or reminding; under the supervision of the RN and/or LPN (CT and MA ONLY). • Other duties as assigned Qualifications • High School Diploma or GED. At least 1 year resident care experience required. • CPR Certification • Certified Nursing Assistant or Certified Home Health Aide or Direct Care Staff Competency exam (PA ONLY). • Knowledge and experience in working with the elderly population, preferred dementia experience. • Able to speak, read and write in English. • Able to work weekends & holidays. Physical Abilities • While performing the duties of this job, the associate is often required to stand, walk, sit, use fine and gross motor skills, reach with hands and arms, balance, stoop, kneel, crouch, talk, hear, and smell. • An individual in this position will be required to lift or carry weight in up to 50 lbs. Intermittent physical activity includes lifting and supporting residents. The associate must use proper body mechanics.
