Registered Nurse (RN) Care Advocate — Patient Navigation

MiraMace is seeking a Registered Nurse (RN) Care Advocate for a remote position focused on patient navigation. In this role, you will support Medicare patients after their initial clinical visit, helping them understand their care plans, schedule appointments, communicate with healthcare providers, and access community resources. This position involves non-clinical navigation support, ensuring patients can effectively follow through with their care without providing medical advice. Candidates should be organized, self-directed, and capable of managing multiple patients simultaneously while maintaining thorough documentation. Comfort with technology is essential, as you will be using electronic health records, customer relationship management systems, and various messaging and scheduling platforms. The role is designed for individuals who are compassionate towards older adults and those facing healthcare barriers, demonstrating patience and respect in all interactions. This is a full-time independent contractor position requiring a minimum commitment of 20 hours per week, with the potential to work up to 40 hours.
